Please read this notice before you provide any Personal Data to us

1. 豐盈動物腫瘤中心(本中心)是一私立動物醫院。本中心的工作人員可能會請你提供你的個人資料,作為有關你的獸醫服務及治療用途。The Harvest Veterinary Oncology Centre (The Centre) is a private Veterinary Centre. Our staff members may ask you to provide your Personal Data for purposes related to veterinary services and treatments.

2. 當你提供這些個人資料時,請確保這些資料準確及完整。When you provide Personal Data to us, please make sure that the data is accurate and complete.

3. 申請人所提供個人的資料將用於獸醫服務及治療有關之事宜上。The information provided by you will be used in our medical record. All information provided will be kept in strict confidence.

4. 並請注意,你的個人資料可能會被本中心交予: Please also note that your Personal Data may be made available to:

* 本中心內有關人士; Appropriate persons in our centre;

* 任何其他有關人等需要該資料作為你提供獸醫服務及治療用途;或 Any other relevant parties who require it for matters related to veterinary services and treatments.

* 任何有關的政府部門/ 管理機構(包括「稅務局」、「勞工署」和「廉政公署」),作 為有關法例的用途和目的。Any relevant government departments/ appropriate authorities including Inland Revenue Department, the Labour Department and ICAC when The Centre is required to provide it under the relevant legislation for use for the purposes of that legislation procedures.

5. 除了上文所述以外,本中心只會在下列情況下把你向本院提供的個人資料使用,透露或

轉移:In addition to the above, we will only use, disclose or transfer the Personal Data which you provide to us:

* 以為你提供獸醫服務及治療的用途或其他直接有關連的目的;或

For those purposes relating to your veterinary services and treatments with The

Centre; or

* 在法律容許的情況下。Where permitted by law

6. 本中心將會在得到你的同意後,才使用你的個人資料作為其他目的。We will obtain your consent before using your Personal Data for any other purposes.

7. 如果你希望根據《個人資料(私隱)條例>>要求查閱和/或改正你的個人資料,請於 「豐盈動物腫瘤中心」的辦公時間內與我們聯絡。If you wish to require access to and/ or correction of your Personal Data, you may do so under Personal Data (Privacy) Ordinance. For request(s) relating to Harvest Veterinary Oncology Centre, please contact the us during office hours.
